Anything you have is useful. Don’t wait until you have everything — partial records are better than no records. Helpful items include:
- Admission agreement and any documents signed at intake
- Any medical records, wound photos, or incident reports you have received
- A written timeline of events as best you can reconstruct it
- Any communications — emails, texts, voicemails — with facility staff or administrators
- Documentation of any complaints made to the facility, the state, or the ombudsman
- Information about your authority to act on behalf of your loved one (POA, guardianship, estate documents)
